Cancellation

Please give us a 'One Day' notice for cancellations. Late Cancellations without rescheduling and 'No Shows' are subject to a $50 charge. Call if you have a question or a concern or if you will be late. We will do our best to fit you in but we need to know in advance for the consideration of other patients.

First Time Patients

• Bring your insurance card and prescription.
• We can not treat patients without a prescription.
• Come 15 minutes early to fill out paperwork.
• Bring comfortable gym clothes and tennis shoes.

In today's health care we are always sensitive to the financial concerns of our patients. We are In-Network with United Health Care and work with all Major Insurance Companies.
